RELEASE
OF LIABILITY
On behalf of my child, I (Parent/Guardian) have voluntarily requested to participate in dance lessons and performances offered by The Red Shoes Performing Arts Studio. I am aware that attending or participating in these activities involves risk of injury to person and property. I voluntarily accept and assume all risk from attending or participating in these activities. In consideration of being permitted to participate in these activities, I agree, on behalf of myself and my child, our heirs, personal representatives and assignees, not to make any claim against or sue The Red Shoes Performing Arts Studio, or their owners, employees, officers, directors, agents, or members (collectively referred to as the "RELEASED PARTIES") for any injury or damage to my child or myself arising from the negligence, or other acts, however caused, of the Released Party. In addition, I release and discharge the Released Parties for all actions, claims or demands that I or my child, our heirs, personal, representatives, or assignees, have or may hereafter have for personal injuries to my child or myself, or property damage resulting from the activities described above. This release includes injury or damaged caused by negligence, active or passive, or other actions of the released parties. I understand that photos are occasionally taken at class or performances by The Red Shoes Performing Arts Studio, and that any photo taken of my child may be used for The Red Shoes Performing Arts Studio publicity purposes. I understand that I will be solely responsible for the cost of transportation to and from all classes and performances, costumes, dance shoes, class materials and / or any other expense as required by The Red Shoes Performing Arts Studio. By checking on
the "I agree" check box, I hereby give my permission for any
necessary medical care to be given to my child in the case of an accident
or illness. I agree to assume full responsibility for the cost of any
treatment provided. |
